Any/All documents submitted by employees for their leave must be forwarded immediately to the Office of Human Resources
Sick Leave
Submission of a sick leave document for more than 3 days requires a Medical Certification Form to be submitted. Submission of Sick Leave documents for less than 3 days is not required.
Submission of vacation leave document s should be reviewed to ensure compliance with system policies and regulations. Approval of vacation leave must be done prior to employee leaving work.
Jury Duty
Submission of this type of leave requires a copy of the Jury Summons or a document from the court showing the day(s) the employee served as a jury member.
Sick Leave Pool
Submission of a Sick Leave Pool leave document requires receipt of the Sick Leave Pool Approval letter sent by the Central Leave Coordinator.
This type of leave is only approved by the Office of Human Resources. If an employee submits this type of leave, the document will need to be rejected. If the employee has been terminated and placed on Administrative Leave, then the department will contact the Office of Human Resources so that the time can be granted and the employee will need to submit leave documents.
Death of a Family Member
Submission of a leave for the “Death of a Family Member” should be 5 days for a spouse, parent, or child and 2 days for brother, sister, grandparent or grandchild
Voting Leave
This leave should only be approved during an election year with prior approval of supervisor.
Unsafe Working or Travel Conditions
Leave documents for Unsafe Working or Travel Conditions will be based on an announcement from the President of the University. If the University is not officially closed the employee will need to use his/her personal leave.
Military Leave
Submission of Military leave requires an official copy of the orders to be sent to the Office of Human Resources. The orders must contain the dates of leave.
Volunteer Fire Fighter/EMS
Submission of leave requires a written statement from the Fire Chief of the Volunteer Fire Department from which the employee completed training including dates of training or duty.
Court Appointed Special Advocates (CASA)
Submission of leave cannot exceed 5 hours per month and a written statement from the CASA Agency enlisting the services of the employee and providing the date and time of service.
Other Emergencies
Submission of this type of leave should be rejected and the employee must resubmit using his/her personal leave.
Suspension with Pay
Submission of this type of leave should be rejected unless a written letter (approved by Office of Human Resources) is provided by the supervisor.
Education Release
Submission of this leave document requires approval by the Office of Human Resources.
Leave without Pay
Submission of this leave document requires a payroll adjustment during the week of the leave until TimeTraq is implemented for all “non-exempt” employees.
